descrição do estudo: Este é um estudo multicêntrico de fase Ib/II, aberto, avaliando a segurança, tolerabilidade, eficácia e farmacodinâmica/farmacodinâmica de APG-2575 em combinação com Pd/DRd em pacientes com mieloma múltiplo recidivante/refratário (RR) ( MILÍMETROS). O estudo consiste em fases de escalonamento de dose e expansão de dose. O estudo consiste em começar com 2 braços indicados abaixo, ambos os braços são independentes.
Braço A: APG-2575 será administrado em combinação com Pd para determinar o MTD e /RP2D de APG-2575 em indivíduos com R/R MM.
O desenho 3+3 será utilizado na fase de escalonamento de dose de APG-2575 em combinação com Pd. A dose alvo inicial de APG-2575 é de 400 mg (nível de dose; DL1) e será escalonada em coortes subsequentes para 600 mg (DL2), 800 mg (DL3) de acordo. A redução da dose para 200 mg (DL-1) é aceitável se o APG-2575 na dose de 400 mg não puder ser tolerado. Este desenho baseado em regras prossegue com coortes de três pacientes. Se nenhum dos três pacientes inscritos no DL1 apresentar DLT, outros três pacientes serão tratados no DL2 e assim por diante. No entanto, se um paciente apresentar um DLT, mais três pacientes serão tratados com o mesmo nível de dose. O aumento da dose continua até que pelo menos dois pacientes entre uma coorte de três a seis pacientes experimentem DLT. O MTD é convencionalmente definido como o nível de dose mais alto no qual ≤ 33% dos pacientes experimentam DLT. O nível de dose mais alto seria considerado após uma análise abrangente dos dados de segurança no contexto de 800 mg pode ser bem tolerado, caso contrário, 800 mg (DL3) deve ser considerado como MTD (Braço A). RP2D (Braço A) será determinado com base na eficácia e perfil de segurança de APG-2575 em combinação com Pd.
Os pacientes receberão APG-2575 na dose alvo uma vez ao dia por 28 dias mais pomalidomida (4 mg) nos Dias 1 a 21 e dexametasona (40 mg para pacientes ≤ 75 anos ou 20 mg para pacientes > 75 anos) nos Dias 1, 8, 15 e 22 de um ciclo repetido de 28 dias

O DLT será definido com base na taxa de eventos adversos de grau 3-5 relacionados ao medicamento ocorridos nas primeiras 6 semanas (2 ciclos) de tratamento do estudo. Estes serão avaliados via CTCAE versão 5.0

Pacientes MM (para Braço A e Braço B): Pacientes com MM Recidivante/Refratário de acordo com os critérios IMWG de 2016, previamente tratados com pelo menos 1, mas não mais de 4 linhas anteriores de terapia para MM. Já o MM refratário é definido como doença que progride na terapia de resgate ou evolui dentro de 60 dias do último tratamento.
Pacientes com amiloidose AL (SOMENTE para o Braço C): Pacientes com amiloidose AL ao atender:
i. histochemical diagnosis based on detection by polarizing microscopy of green birefringent material in Congo red-stained tissue specimens, the type must have been confirmed unequivocally.
ii. have symptomatic organ involvement. Only purpura and/or carpal tunnel syndrome are not acceptable.
iii. have at least one prior line of systemic therapy for AL. Patients who do not achieve at least a PR to frontline therapy in 3 months are eligible.
iv. All MM/AL patients should have measurable disease of AL amyloidosis as defined by at least ONE of the following:
Serum monoclonal protein ≥1.0 g/dl by protein electrophoresis
>200 mg of monoclonal protein in the urine on 24-hour electrophoresis

ANC ≥1.0 x 10^9/L independent of growth factor support within 7 days of the first dose with study drug
Hemoglobin ≥8 g/dL without transfusion or growth factor support within 7 days of the first dose of study drug
Platelet count ≥ 50 x 10^9/L without transfusion support within 7 days of the first dose of study drug (for MM patients); or platelet count ≥ 100 x 10^9/L or ≥ 50 x 10^9/L if bone marrow involvement independent of transfusion support in either (for AL amyloidosis patients).

Male and female subjects who agree to use highly effective methods of birth control (e.g., condoms, implants, injectables, combined oral contraceptives, some intrauterine devices[IUDs], sexual abstinence, or sterilized partner) during the period of therapy and for 90 days after the last dose of study drug
Ability to complete questionnaire(s) by themselves or with assistance (For AL amyloidosis patients only)

Subject has received antineoplastic therapy within 2 weeks before the date of initiating study treatment
Subject has previously received an allogenic stem cell transplant (regardless of timing)

For Arm A/C only: The subjects show evidence of intolerance to pomalidomide, which is defined as subjects discontinued due to any AEs related to prior pomalidomide treatment
For Arm B only: The subjects show evidence of intolerance to daratumumab or lenalidomide, which is defined as subjects discontinued due to any AEs related to prior daratumumab or lenalidomide treatment
Patients with any uncontrolled active systemic infection, including but not limited to active hepatitis B or C virus infection, known human immunodeficiency virus (HIV) positive
Subject has peripheral neuropathy ≥grade 3 except caused by AL amyloidosis
Subject has plasma cell leukemia (>2.0*10^9/L circulating plasma cells by standard differential) or Waldenström's macroglobulinemia or POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, and skin changes)
Plasmapheresis <35 days prior to the initiation of study drug
Failure to have fully recovered (i.e., ≤ Grade 1 toxicity) from the reversible effects of prior treatment for MM or AL amyloidosis
Unable to swallow tablets or malabsorption syndrome, disease significantly affecting gastrointestinal function, or resection of the stomach or small bowel, symptomatic inflammatory bowel disease or ulcerative colitis, or partial or complete bowel obstruction
Currently active, clinically significant cardiovascular disease, such as uncontrolled arrhythmia (including Frederica corrected QT interval (QTc) ≥470 msec ) or Class 3 or 4 congestive heart failure as defined by the New York Heart Association Functional Classification; or a history of myocardial infarction, unstable angina, or acute coronary syndrome within 6 months prior to initiating study treatment
Major surgical procedure within ≤14 days prior to initiating study treatment, or anticipation of the need for major surgery during the course of the study treatment, radiotherapy ≤14 days prior to initiating study treatment, systemic treatment within 14 days before the first dose of APG-2575
Recent infection requiring systemic treatment that was completed ≤14 days before the first dose of study drug
Vaccinated with live, attenuated vaccines within 4 weeks of initiation of APG-2575 (for patients' safety, patients could receive COVID-19 vaccination before or during study period as judged by HCP as beneficial)
Subject has any concurrent or recent malignancy ≤ 1 year prior to registration with the exception of: basal or squamous cell skin cancer, any carcinoma in situ. NOTE: If there is a history or prior malignancy, they must not be receiving other specific treatment for their cancer.
Requires treatment with a strong cytochrome P450 (CYP) 3A inhibitor
Known bleeding disorders (e.g., von Willebrand's disease) or hemophilia
History of stroke or intracranial hemorrhage within 6 months prior to enrollment
